Service Description

Any treatment and training session continuing the treatment plan for the complaints assessed during the Initial Physical Therapy Session and within 30 days of initial consultation or last treatment session. If 30 days have past, or there are new complaints, clients will need to schedule a re-evaluation appointment. If your plan of care requires less than 1 hour, the rate will be prorated at $35 each 15 minute unit of time with a 30 min minimum. Mobile service is available within a 5 mile radius of my clinic for a $30.
