Service Description

A thorough history and initial assessment of your area of concern, goals, physical therapy diagnosis, discussion of treatment options, trial treatment, and treatment planning. Most evaluations take 60-90 minutes depending on the complexity of your condition and how many areas are involved. It is usually advisable to stick to one or two issues that are most functionally limiting, and expand to other areas once we have an effective plan of care in place for your primary concerns. I will be assessing for the root causes and drivers of your impairments, so it is not unusual to treat a dysfunctional area that does not cause you any pain. A detailed patient history form will be sent prior to your first appointment to save time taking your subjective and allow me to review and plan your evaluation. Additional subjective assessments may be sent for you to complete after reviewing your history. I am currently contracted with Blue Cross of Idaho, Cigna, Regence Blue Shield, and Medicare. If you are private pay, or I am non-contracted out of network with your insurance, I require payment in full at the time of service. If you have insurance coverage for physical therapy, you will be provided with a superbill that can be remitted to your carrier for reimbursement. Please contact your insurance carrier to determine coverage, if a physician's referral and pre-authorization are required before scheduling to make sure your evaluation is covered. If you a managed plan, such as a Medicare Advantage plan, that permits you to see out of network providers and submit for reimbursement, I should be able to provide your service. If you have straight Medicare, I am required to submit physical therapy service charges on your behalf, but there is no guarantee that Medicare will cover the cost of your care. Please check with your member services to verify your benefits and terms of service. Mobile Service is available within a 5 mile radius from the clinic for an additional $30 per session which is not billable to insurance, as it is a convenience fee. Mobile outpatient services are not the same as home health services.
